The Arc seeks 1:1 direct care support staff for multiple program members. The ideal candidate will facilitate and support an assigned caseload by engaging in daily activities both in the home and out in the community for recreation, leisure, personal appointments (including transportation to work) and support ISP goals and objectives.
Part time or Full Time openings. Weekend and evenings a must.
Responsibilities:
= Attend to the daily needs and assist with all activities of daily living and/or skill building
= Create and provide opportunities for engaging leisure time activities both in the home and out in the community and encourage independence in the choice and participation of those activities
= Provide support by ensuring that daily activities, plans and routines reflect preferences, needs and goals
= Records and submits weekly progress notes to the IHS/SL Program Coordinator on a weekly basis
= Provides on going transportation for program members to appointments, social/recreation events, jobs, errands and promotes overall meaningful community access. (personal car must be used, mileage is reimbursed)
= Frequently and accurately communicate with family members, residential providers and the IHS/SL Program Coordinator and Program Director
= Foster and promote Human Rights and implement any and all written clinical or individualized plans and protocols (Positive Behavior Supports, Behavior Support Plan, specialized diets, Incident reporting, medical protocols)
= Other duties as assigned by the IHS/SL Program Coordinator or Program Director
Qualifications desired:
= Prior experience and passion working with people with disabilities is preferred
= Comfortable assisting with personal care
= Ability to transport safely (personal car must be used, mileage is reimbursed)
= Positive attitude and ability to work independently as well as ability to work across multiple different settings- must be able to embrace changes within caseload and work settings
= Effective critical thinking and communication skills
= Ability to compose accurate data in real time
= Windows, iOS, Microsoft Word and Outlook experience required
= Must demonstrate professionalism at all times
= Ability to lift, push, pull, maneuver up to 25 pounds
= Must possess a valid driver’s license with clean driving record
= Must be able to pass criminal background checks
= High School degree or equivalent required

The Arc of Plymouth and Upper Cape Cod is seeking Shared Living Providers who are interested in welcoming an adult with intellectual/developmental disabilities into their home. As a Shared Living, you will explore opportunities for providing community integration through social and recreational activities that the person is interested in. It will be the provider’s responsibility to give individualized support to the person on a daily basis. A Shared Living provider is providing care in their own home like: meals, supporting activities of daily living, promoting relationships and friendships, overseeing medical care, transporting to activities, etc. The provider should be committed to supporting The Arc of Plymouth and Upper Cape Cod’s mission to support a person with intellectual and developmental disabilities to belong, contribute and thrive.
Shared Living Provider Requirements
-Must be 21 years of age or older
-High School Diploma or GED Equivalent
- Valid Driver’s license and own vehicle
-Ability to pass CORI check, RMV check, and fingerprinting check per DDS regulations
-Ability to become CPR/1st Aid certified
-Good verbal and written communication skills
-Must be willing to provide on-going transportation for appointments, social gatherings, community access etc.
-Must have a separate bedroom in a home that meets all DDS and Arc regulations
-Must be willing to embody The Arc/Shared Living philosophy- to welcome and assimilate the individual into your home, family and community while fostering and promoting growth, human rights and community involvement as well and safeguarding heath and safety
-Experience with people with disabilities desired; knowledge of Positive Behavior Support and Safety Care Certification desired
